Before You Begin

What Working With Dr. Kilgore Looks Like

A transparent guide to the consulting relationship — so you can decide if this is the right fit before we ever speak.

01 — My Philosophy

I Am Your Guide, Not Your Doer

I practice what I call physician-guided health optimization. That means I bring the analysis, the pattern recognition, the data interpretation, and the evidence-based recommendations. You bring the commitment to implement them.

This is not concierge medicine where I manage your prescriptions. This is not wellness coaching where I give you motivational pep talks. This is a structured advisory relationship where a board-certified internist, CDC-trained epidemiologist, and member of the American College of Lifestyle Medicine applies quantitative analysis to your biology — and then tells you what the data says, even when it is uncomfortable.

My work is grounded in the evidence-based framework of lifestyle medicine — the clinical discipline that uses nutrition, physical activity, sleep, stress management, avoidance of risky substances, and social connection as first-line interventions for chronic disease prevention and reversal. Every recommendation I make is guideline-concordant and rooted in peer-reviewed research, not trends or proprietary protocols.

I will always tell you what your data reveals, not what you want to hear. I will challenge assumptions. I will push you toward evidence over intuition. And I will celebrate real, measurable progress when your biomarkers prove the strategy is working.

— Dr. Paul Kilgore, MD, MPH, FACP

02 — How I Work

Four Principles That Define Every Engagement

📊

Data First, Always

Every recommendation I make is grounded in your actual biomarker data, genetic results, and wearable metrics — not trends, not guesswork, not what worked for someone else.

🎯

Episodic & High-Impact

Our sessions are focused and strategic. I drop in with deep analysis and clear direction, then you execute. I am the liberating force, not the occupying force.

🔬

Proprietary Tools

I have built 28+ health analysis tools — from Phenotypic Age and CardioGuard to OvarySpan and CancerClarity — that give you insight no standard doctor visit provides.

🤝

Partnership, Not Dependence

My goal is to make you increasingly capable of interpreting your own health data. The best outcome is that you need me less over time, not more.

03 — Honest Fit Assessment

Is This Right For You?

I would rather you know now whether we are a good match than discover it after you have invested time and money. Please read both columns carefully.

✔ This Is For You If…

  • You are willing to get comprehensive bloodwork done on a regular schedule
  • You want a physician who interprets data, not a coach who gives generic advice
  • You are ready to hear uncomfortable truths about your biology
  • You will follow through on recommendations — supplements, lifestyle changes, retesting
  • You value evidence and measurement over anecdotes and fads
  • You understand that meaningful biomarker change takes 3–6 months of consistent effort

✖ This Is Not For You If…

  • You want quick fixes or miracle supplements
  • You are looking for someone to manage your medications or replace your primary care physician
  • You are not willing to get lab work done
  • You want someone who will tell you what you want to hear
  • You expect me to do the implementation work for you
  • You are shopping for the lowest price — this is premium, expert-level advisory work

04 — Your Responsibilities

What I Expect From You

A consulting relationship only works if both sides do their part. Here is my commitment and yours:

My Commitment to You

I will prepare thoroughly for every session. I will give you my honest, evidence-based assessment. I will respond to communications within the timeframe specified for your tier. I will continuously update my analysis as new data comes in. And I will tell you when something is outside my scope and refer you to the right specialist.

Your Commitment to the Process

Get your bloodwork. This is non-negotiable. Without current lab data, I cannot help you. I will tell you exactly which panels to order and where to get them at the best price through our lab partner.

Complete the intake form before our first session. The more detail you provide, the more targeted our time together will be. Do not show up unprepared.

Show up on time. I hold a 6-minute window. If you have not joined by then, we reschedule to the next available slot. My time is limited and I protect it so that I can give every client my best thinking.

Do the work between sessions. I provide the roadmap. You drive the car. If you are not implementing the recommendations, there is little point in continuing.

Communicate honestly. Tell me if something is not working. Tell me if you cannot afford a recommended test. Tell me if you stopped taking a supplement. I cannot help you if I am working with incomplete information.

05 — The Journey

What Your First 90 Days Look Like

Before We Begin
Onboarding & Lab Work
You receive your Welcome Guide, Client Intake Form, and Lab Ordering Guide. You order and complete your bloodwork. You fill out the intake form thoroughly and submit it before our first session.
Week 1–2
Deep-Dive Interpretation Session
Our first session focuses entirely on your data. I run your results through my proprietary tools, identify your key risk areas, calculate your Phenotypic Age, and build your initial optimization protocol.
Week 3–6
Protocol Implementation
You begin implementing the recommendations — targeted supplementation, lifestyle modifications, exercise adjustments. We check in on adherence, troubleshoot barriers, and fine-tune the approach.
Week 8–10
Follow-Up Labs & Reassessment
You retest key biomarkers. We compare to baseline, measure what moved, identify what needs adjustment, and celebrate measurable progress. This is where the data proves the strategy.
Month 3+
Optimization & Expansion
With your foundation solid, we begin expanding into deeper layers — genetic interpretation, epigenetic markers, wearable data integration. Your protocol becomes increasingly precise and personalized.

06 — Working Terms

Communication & Logistics

Sessions

All sessions are conducted via HIPAA-compliant Zoom telemedicine. I have a signed Business Associate Agreement with Zoom Communications, Inc. Your data and our conversations are protected.

State Licensure & Where I Can Practice

I currently hold active, unrestricted medical licensure in Michigan and Alabama, with applications pending in Pennsylvania, Wisconsin, and Missouri. Telemedicine consultations are provided only to patients physically located in a state where I hold active licensure at the time of our session. This is how telehealth is regulated in the United States — the law looks at where you are, not where I am.

If you live in a state where I am not yet licensed, you have two options. First, check back — I continue to expand my licensure as practice needs evolve, and your state may become available. Second, I offer a range of non-clinical resources that do not require a physician-patient relationship: courses, ebooks, health-tracking templates, and the 26+ free health tools on my platform. These are available to everyone, regardless of location.

I take this seriously because you should. A physician who casually offers telemedicine across state lines without proper licensure is operating outside the legal and ethical standards of medicine — and that is not a physician you want interpreting your health data.

Scheduling

You book sessions online through my scheduling system. A 15-minute buffer follows every session so I can complete my notes and your follow-up recommendations while our conversation is fresh.

Between Sessions

Essential clients communicate via email (48-hour response). Comprehensive clients receive priority email (24-hour response). Executive clients have direct message access to me personally. I do not use intermediaries or assistants for client communication.

Payment

All consulting tiers are billed monthly in advance. You may cancel anytime — there are no long-term contracts. HSA and FSA funds are eligible for payment. I require payment before the engagement begins because it preserves my objectivity — I never want financial considerations to influence the honesty of my recommendations.

Confidentiality

Everything we discuss is confidential. I will never share your data, your identity, or your health information with anyone without your explicit written consent. If I reference patterns from my client work in published content, it is always fully de-identified and aggregated.

07 — Common Questions

Questions Prospective Clients Ask

Do you provide references from current clients?+
I do not routinely provide client references, for two reasons. First, my clients have not signed up to advise you — they hired me for their own health optimization. Second, all of my client work is confidential. What I can offer: my professional credentials are publicly verifiable (ABIM board certification, Wayne State University faculty appointment, CDC Epidemic Intelligence Service training), and my published research speaks for itself. I am also happy to do a brief complimentary discovery call so you can assess the fit firsthand.
What are the typical deliverables?+
My primary deliverable is insightful analysis and evidence-based recommendations. You will receive personalized reports generated by my proprietary tools, a clear action plan after each session, and benchmark comparisons showing where you stand relative to optimal ranges. I do not produce lengthy generic reports. Everything I deliver is concise, specific, and actionable.
How is this different from my regular doctor?+
Your primary care physician manages disease. I optimize health. The average doctor visit is 12 minutes and focuses on whether you meet the criteria for a diagnosis. My work focuses on the quantitative space between “not sick” and “truly optimized” — using deeper biomarker analysis, validated aging calculators, genetic and epigenetic data, and wearable metrics that your regular physician does not have the time or tools to interpret.
What if I disagree with a recommendation?+
Good. That means you are thinking critically. I will always explain the evidence behind my recommendations. If you disagree, we discuss it. I hold my conclusions subject to modification when presented with good reason. What I will not do is tell you what you want to hear. If the data says something, I will say it — kindly, but clearly.
Can I start with a single session before committing to monthly consulting?+
Yes. The $225 Physician Interpretation Session is designed for exactly this. You bring your lab results, I run them through my tools and give you a personalized analysis and action plan. If we are a good mutual fit, we can discuss ongoing consulting from there. There is no pressure and no obligation.
How quickly will I see results?+
Most clients see measurable biomarker improvement within 8–12 weeks of consistent implementation. Some markers move faster (inflammation markers like CRP), some take longer (metabolic markers, biological age). I set realistic expectations from Day 1 and we track progress with follow-up labs at defined intervals. This is not a quick fix — it is a systematic, data-driven optimization of your biology.
Is this a substitute for my primary care physician?+
No. I complement your existing medical team. I do not prescribe medications, order imaging, or manage acute illness. I interpret your data through a health optimization lens that most physicians do not have the time or training to apply. For Executive clients, I will coordinate directly with your other providers to ensure alignment.
